Official Communications for Current Bingham & Taylor Team Members
The Affordable Care Act (ACA) requires employers to offer health insurance coverage to full-time employees and their dependents. In compliance with ACA requirements, B&T provides an annual statement, Form 1095-C, to all eligible employees detailing the insurance coverage available to them and their dependents. The Form 1095-C for the 2025 tax year is now available.
Right to Request a Copy
Form 1095-C is available online. You can access the form by logging into UKG Ready and selecting the three lines icon in the top left-hand corner and navigating to My Info > My HR > Forms > Government Forms and selecting the 1095-C option.
You also have the right to request that a copy of Form 1095-C be mailed to you. If you request a copy, a copy will be mailed to you within 30 days. To request a copy of the form 1095-C, email lfrazier@bandt-us.com or mail your request to:
